The Fourth Doctor's huge scarf was the result of a misunder- standing. Doctor Who head costume designer Jim Acheson wanted the Fourth Doctor to have a scarf, so he gave a knitter lots of wool in many colors and asked her to make one. Apparently, she thought she was supposed to use it all, and knitted a multicolored, freakishly huge scarf. The Muppet Show

Elmo's unfilmed scene in the 2011 Muppets reboot turned out to be pretty meta. The scene had Elmo being asked to be The Muppet Show's celebrity host, and a team of lawyers stepping in to nix that. The Golden Girls
